for choosing our practice. We are committed to the success of your treatment
and care. Payment of your bill is part of this process. This policy explains
our expectations for payment of office services.
Payment for Office Visits and
expected at the time services are rendered unless other arrangements are made. Our
practice accepts payment in the form of cash,
check, Visa, Mastercard and Discover.
We will call
all insurance companies to verify your benefits to the best of our ability.
Once we know what type of coverage you have we will estimate benefits to
establish your co-pay. Please be advised
that this office makes no assurance, inferences, nor guarantees regarding your
insurance coverage. Although we may participate with an insurance carrier, it
is impossible for this office to be aware of, nor versed in, each particular
plan’s coverage, as there are a multitude of insurance plans and coverage
particulars. Each plan is different and the contract negotiated by your
employer may contain restrictions that others do not. You are responsible to
know these restrictions.

to verify any insurance coverage regarding treatment in my office. It is your
insurance coverage – it is your responsibility to ascertain benefits as well.

provider for dental plans: Delta Dental
PPO, Preferred and Premier, Met Life, United Concordia, Cigna Radius, Anthum Blue Cross
Blue shield, and Blue Cross Blue Shield 100 plans. We will work with all other dental insurance
companies (ie: Aetna, ect.) for
you to obtain the maximum benefit allowed from your policy. Dr Moccio is NOT
participating with any medical HMO plans.
patients who are not able to pay in full on the day services are rendered we
offer a payment plan option. Our policy is to collect 50% of the patient’s
responsibility on the date of service. We will work with you to set up a 2 or 3
month payment plan to finalize the balance.
